DATE OF PARTY RENTAL ___________________  (Insure date is entered on Post 490 Calendar)

1. Hall rental fee is $100.00.  Use of kitchen facilities, additional $25.00
(POST 490 MEMBERS MAY BE SUBJECT TO A 50% FEE DISCOUNT) ________(INITIAL)

2. Pavilion rental fee is $200.00  (Includes the pit for cooking) Use of the pavilion kitchen is an additional; $25.00.___________ (INITIAL)

3. Combined Hall and Pavilion rental fee is $350.00 w/outside bar.______(INITIAL)

4. A refundable security deposit of $25.00 (inside area) and $200.00 (outside area) is required. _________(INITIAL)

5. Facility cleanup is the reseponsibility of the renter. ________(INITIAL)

6. The security deposit of $25.00 (inside area) and $200.00 (outside area) will not be refunded until a physcial inspection of the rented area and facilities by a designated VMHA member and renter is completed.                         _________(INITIAL)

7. Date requested will be reserved upon receipt of application deposit.   Deposit is non-refundable in the event of a no-show or cancellation._________(INITIAL

8. Renter is responsible for all persons or property resulting from or alleged to have arisen from the conduct of the persons at or associated with the rental activities.  Renter is responsible for all acts or omissions of renter's family, agents, employees, guests or invitees.

**  The Renter agrees to indemnify, hold harmless and defend the Veteran's Memorial Home Association (VMHA) and Post 490 from and against any and all claims, demands, actions, liabilities, judgements, cost and attorney fees arising out of claimed on account of, or in any manner predicated upon loss of or damage to rental property and/or injuries to or death of any and all persons (to include, but not limit to, renter's family members, agents, employees, guests or invitees), in any manner caused by or contributed to by renter, renter's family members, agents, employees, guests, or invitees. ___________ (INITIAL)

9. Time use limit on the facilities is 1:00 AM __________ (INITIAL)

10. Minors are prohibited from consuming alcoholic beverages inside or outside.
    Enforcement is the sole responsibility of the renter. _________ (INITIAL)

11. All beverages for on the premise consumption MUST be purchased from the VMHA.  (Beer, Wine, Soda, Liquor, etc.)  ____________ (INITIAL)
                           Current Prices are as follows:      $80 per half keg of Genessee
                                                                                $90 per half keg of Bud or Coors
                                                                       Bar chairperson will price quarter kegs
                                                                              $35 per canister of soda, any kind
                                                                   All wines are charged at market plus prices.

12.  This is a CLUB.  Smoking is allowed in the bar area.   Smoking in the back room or the outside pavilion is the responsibility and at the discretion of the renter.

13.  I have read the contents of this rental agreement and fully understand it.  I, as the signed renter, WILL BE ON THE PREMISES AT ALL TIME DURING THE RENTAL.  Any special commitments will be written here at the signing of the agreement.
